Authorization of Funds
Title IV funds are administered by the U.S. Department of Education and include: Federal Pell Grant, Federal Supplemental Educational Opportunity Grant or SEOG, Direct Loan (Subsidized, Unsubsidized, and PLUS – Parent or Grad), TEACH Grant, Perkins Loan.
Federal regulations require that Title IV funds be automatically applied to allowable fees such as tuition, room and board, and mandatory fees. However, Mid-America Christian University (MACU) must obtain your written authorization to apply your Title IV financial aid to all other charges appearing on your student account such as parking fee, book charges, health insurance, and other miscellaneous charges.
To enable MACU to apply excess Title IV funds to these other charges, you must select “YES” on the Authorization statements below.
If you select “NO” or do not complete the form, all outstanding or future charges will remain your responsibility. A financial hold will be placed on your account for any outstanding balance that will prevent future course registration or transcript releases until the account is paid in full.
This is a voluntary authorization and is valid from the date of submission for all periods of enrollment at MACU. You may rescind/revise this authorization by giving written notice to the Office of Student Financial Services (will not be retroactive).
I authorize Mid-America Christian University to apply Title IV funds to all outstanding charges to my student account for the current year.
I authorize Mid-America Christian University to apply Title IV funds to prior year charges up to $200.00.
